Speedoc’s Chronic Disease Home Management (CDHM™) programme is a fully home-based treatment programme designed to help you manage your chronic conditions optimally, right from the comfort of your home. All our CDHM™ programmes are strictly guided by the Ministry of Health’s clinical practice guidelines. Chronic conditions we treat under CDHM™ include:

This programme is fully customisable to your medical history and needs. Took a urine test just a month ago and don’t want to take it again? Let us know and we’ll remove it from the list of tests you’ll receive as long as you still have the results with you. Need an additional blood test for your thyroid? Add it to the list. Our Patient Care Team will listen to your needs and help you craft your very own CDHM™ programme.
Our CDHM™ programme for hypertension includes:
All relevant lab tests
One (1) doctor house visit accompanied by a nurse
Three (3) video consultations with a doctor to review your condition
You will also receive five (5) additional video consultations that can be used by any family member, for any condition, within the 12 months you’re enrolled in the programme.
You will have a total of three (3) consultations with your doctor:
Your doctor will visit you at home with a nurse to conduct the necessary tests and examinations to set a baseline for your treatment for the upcoming year.
Approximately three to five days after your first visit, your doctor will schedule a follow-up video consultation with you to go over your test results and clarify any doubts you may have. Six months’ worth of medication will also be prescribed and delivered to you, free of charge.
Your review will be done six months after your initial consultation. Our Patient Care Team will contact you ahead of time to schedule your review. This consultation will be done over video consultation, where your doctor will review your condition and make the necessary adjustments to your medication if needed. Another six months’ worth of medication will be prescribed and delivered to your home. You will have the option of renewing your enrolment after this consultation.
The danger of hypertension is that it often goes unnoticed. There are no indicative symptoms until it is very severe.
In general, you may be at higher risk of hypertension if you:
Smoke
Have a higher body mass index (BMI)
Lead a sedentary lifestyle
Have a strong family history of hypertension
In the unlikely event that your blood pressure gets extremely high, you may notice the following symptoms:
Severe headaches
Nosebleeds
Fatigue
Pounding in your chest, ears, or neck
Chest pains
Difficulty breathing
The best way to find out is through regular checks and self-monitoring at home with a blood pressure monitor. This is why we schedule follow-ups with you under the CDHM™ programme to ensure that your blood pressure is under control.
Poorly controlled high blood pressure could cause serious complications like stroke, heart disease, vision damage, and even kidney failure. This may increase your individual healthcare costs due to hospitalisation and treatment of further complications.
